Cortex Partnership Points Program

Terms of the Partnership Points Program

1. Membership in Partnership Points will be granted by Cortex Medical Management Systems (CMMS) through registration as a Cortex Partnership Points Customer.
2. Partnership Points will only be awarded starting with the date of registration.
3. Partnership Points will be awarded for the following:
   • Referrals resulting in a demonstration of Cortex one or more applications - 4,000 points, + 4,000 more points if a contract is signed with new customer
   • Hosting Site Visits - from 1,000 - 12,000 points depending on the length of the visit
   • Acting as a Reference - 1,000 points
   • Creating documentation and presenting a User Group Session - 1,000 points
   • Participating as a User Group Board Member - 2,000 points
   • Conducting Webinars - 2,000 points
   • Contributing Reports - 1,000 points / per report that become part of the base system
   • Provide a Testing Database - 2,000 points
   • Testing New Releases - 2,000 points
   • Including Cortex name/logo on your marketing materials (points awarded with each incident) - 1,000 points
   • Including Cortex name/logo on the front page of your website (points awarded once per year, not per incident) - 1,000 points
   • Including Cortex name/logo on the front page of your website with a link to Cortex website (points awarded once per year) - 4,000 points
   • Participating as a Case Study (testimonial) - 2,000 points
4. To obtain credit for Partnership Points, within 30 days of the occurrence, you must send an email to Judith Krebs, The Customer Advocate (Email Judith) requesting your points and the reason for the credit.
5. Partnership Points are valid for 24 months from the date earned. Your Partnership Points will contain only valid points awarded within the immediately preceding 24 consecutive months. If points are not used within 24 months of the date earned, they will be deleted from your account balance. CMMS reserves the right to cancel the membership of any Member with no credits in the account and who has earned no points for at least 12 consecutive months.
6. Membership in the Partnership Points program is reserved for customers of CMMS only. Being current on maintenance fees is a requirement and past due accounts may be subject to losing any points accrued. If the customer is not on a maintenance plan, then points cannot be redeemed.
7. Partnership Points are not transferrable.
8. Partnership Points can be redeemed as credit toward
   • Invoices incurred when your account goes beyond the allotted 2 hours per month of consultation services
   • Custom projects (i.e., Work Authorization Service Agreements)
   • On site consultation time
9. Partnership Points cannot be redeemed travel and per diem expenses.
10. Partnership Points cannot be used for taxes if levied.
11. Partnership Points cannot be redeemed as credit toward regular monthly maintenance invoices.
12. CMMS reserves the right to amend, suspend, or terminate this program at any time, with 30 days' notice.
